Thank you for your choosing Well Adjusted Chiropractic for your wellness needs. Please complete the new patient paperwork completely and accurately before arriving for your first appointment. If you are unable to complete the paperwork prior to the day of your appointment, please arrive 30 minutes early to fill it out in the office. If your new patient paperwork is incomplete at the time of your appointment, you may need to reschedule. We are sorry for any inconvenience this may cause.
