Welcome to our online Patient Intake Form and thank you for choosing Ball Chiropractic Center! Here at Ball Chiropractic Center we believe the patient's health needs are the forefront of everything we do so as a way to facilitate that we offer a "no charge" consultation. We want to give you the patient the opportunity to see if chiropractic would suit your bodily needs. We ask that at the 1st office visit (if case is not a Personal Injury or Worker's Comp) that 25% of the charges be paid at the time of service.
